PASCO is currently seeking a compassionate and dedicated RN to provide personalized one-on-one care for a 27 years old client in Colorado Springs (Falcon).   This requiring respiratory and GB cares. This dayshift position can be part or full-time and provides the chosen candidate with a flexible schedule.  Typically shifts needed are either 10 or 12 hours in duration (i.e. 8a - 6p). 


POSITION
Private Duty Nurse (PDN)


POSITION PURPOSE

Provide direct, comprehensive nursing services for a single client on an hourly basis. Additionally, this role will provide client education, complete and submit all required documentation, and may supervise or direct other assigned care providers.

KEY DUTIES & RESPONSIBILITIES

  • Provide direct and comprehensive skilled nursing services on an hourly basis as per the client’s physician's orders and in alignment with accepted standards of practice and governing body regulations.
  • Promptly and accurately interpret and execute physicians' orders
  • Conduct assessments encompassing physical, emotional, psychosocial, and environmental aspects of the client's needs, initiating suitable preventive and rehabilitative nursing procedures
  • Aid clients in pursuing care-related goals and recommend goal revisions based on changes in the client's status
  • Foster and sustain relationships with assigned clients and their natural supports in-home
  • Ensure the delivery of high-quality, consistent care by effectively communicating and coordinating with the RN Case Manager, family, physician, and other PDN staff
  • Maintain in-home charts and medication sheets, updating and communicating changes with the RN Case Manager following PASCO policies and governing body regulations
  • Review and report the outcomes of laboratory tests and other diagnostic procedures to relevant care providers
  • Engage in client case conferences with the RN Case Manager, actively communicating pertinent information
  • Demonstrate a comprehensive understanding of the client’s individualized Plan of Care (Physician’s Plan of Care, Home Health Aide Plan of Care), evaluate practices and outcomes against this plan, and recommend changes or revisions to the client, RN Case Manager, and/or physician when appropriate
  • Complete and submit all necessary documentation adhering to governing body rules, regulations, and established Agency guidelines
  • Supervise and direct assigned LPNs and aides in accordance with Federal, State, and Local regulations and Agency Policy
  • Provide guidance on appropriate procedures to LPNs, CNAs, or other care providers as needed
  • Uphold care quality standards and process CNA supervisory forms within specified timelines
  • Contribute to the performance management process by offering feedback
  • Identify the learning needs of clients and/or caregivers, educate them (if applicable), and make suitable referrals to community resources to enhance client health and well-being
  • Maintain open and effective communication with team members and PASCO nursing administration
  • Provide crucial information to the Nursing Supervisor regarding wounds, infections, hospitalizations, etc.
  • Attend nurse meetings, actively engage with information presented, and seek additional knowledge as needed
  • Proactively communicate any clinical questions, compliance queries, special circumstances, or other issues
  • Comply with all HIPAA regulations, ensuring strict confidentiality of client healthcare records
  • Adhere to State and Federal Agencies' rules and regulations as well as PASCO’s administrative guidelines
  • Perform other duties as assigned by PASCO’s nursing administration
  • Access to a reliable transportation, valid Colorado driver’s license and active insurance

Job requirements

EXPERIENCE

  • 1-3+ years experience practicing as a PDN providing in-home care (preferred)
  • Documented experience and skill set matching client/family needs
  • Home Health nursing experience (highly preferred)
  • Active and current drivers license/insurance

EDUCATION/LICENSES

  • Current license to practice as a Registered Nurse in the state of Colorado (confirmed through DORA)
  • Current CPR Certification

KNOWLEDGE/SKILLS/STRENGTHS

  • Clinical Judgment: Ability to independently collect and interpret data about a client, arrive at a conclusion and identify the appropriate action. This involves problem solving, decision making and critical thinking rooted in clinical knowledge and experience.
  • Client Service: Ability to establish and maintain relationships with clients, and provide professional appropriate, compassionate and empathetic care to clients
  • Maintaining Commitments: Ability to consistently fulfill commitments, show up when promised, and maintain strong communication
  • Collaboration: Ability to maintain a positive attitude, demonstrate flexibility and balance self-interest with the interests of the agency
  • Knowledge of State, Federal and Licensing rules and regulations for care delivery and documentation by Home Care Agencies
  • Knowledge of unique issues related to care delivery for people with a variety of disabilities
  • Ability to assess data reflective of the client’s status
  • Ability to communicate effectively both orally and in writing
  • Ability to operate a computer, familiarity with Microsoft products, specifically Outlook, and ability to receive and process work in an Electronic Health Record (EHR) system

PHYSICAL REQUIREMENTS

  • Medium detailed work:
    • Requires walking or standing a significant amount of the time during shift
    • Capability to assist client with mobility or transfers
    • Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
    • Requires stooping, kneeling, crouching, grasping, lifting, reaching, pushing, twisting at waist
    • Requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; viewing a computer terminal; extensive reading; visual inspection involving small defects, small parts, and/or operation of machines (including inspection)

JOB TYPE: part or full-time

SHIFT SCHEDULE: Day shifts. Part of full-time availability working typical 10 hours shifts. 

LOCATION: Colorado Springs, CO 

PAY RANGE: $23 - $38/hour DOE and licensure. The chosen candidate will also receive a $5,000 SIGN-ON BONUS
